Imagination Place! Site Feedback Form: This feedback form helps the project improve activities and should be completed by design club leaders on a weekly basis. Your honest feedback is greatly appreciated! Weekly for duration of your club
Imagination Place! Participant Attendance Sheet: This form helps you keep track of who attends and when they attend. It also helps the project collect important information about who it is reaching. This should be printed out and returned at the end of your club period. Complete every session
Imagination Place! Participant Invention Inventory: This form is provided so that you can keep a record of the work that children have actually published online in KAHooTZ. This helps both you and the project staff to keep track of the online work children have created over time. End of Club/Class _______________
Imagination Place! Critical Incident :This is an essay for design club leaders to complete at the end of their clubs. In a few paragraphs, we ask you to describe some critical design and invention learning experiences that you witnessed children having with Imagination Place! End of Club/Class _______________
Imagination Place! Design Club Leader Evaluation Form: This form helps the project gauge which activities are useful to design club leaders and which ones may need to be improved upon or dropped. It is to be completed at the end of your club by you on the web. End of Club/Class _______________

Imagination Place! Sign-up Form: To be eligible to participate, this form should be completed by students at the start of your club. This form can be completed directly on the web. Start of Club/Class
Imagination Place! Parent/Guardian Consent Form: This form needs to be printed and signed by participantsŐ parents and returned to Imagination Place! This is required by our funders and must be returned for children to be eligible for online accounts. Start of Club/Class
Draw an Inventor - A or B:These tasks should be fun for children to complete. There are two versions of this task -- Form A and B that need to be completed at the start and end of your club. This activity is designed to assess children's conceptions of inventors and how Imagination Place! influences their ideas about who can invent. Start of Club/Class

The Role of Research in This Project

The role of research in this project is crucial! Research helps:

  • the project continually improve resources so they respond to the needs of you and your children
  • provides insights into what children learn and get out of the experience
  • helps secure future funding to keep these efforts going

Research is impossible without your help- the people and children who are actively working with Imagination Place! To ensure that Imagination Place! can be the best it can be, club leaders participating as field test partners are required to complete feedback forms and some surveys. In addition, children who attend club leaders' workshops are required to complete some activities and forms. The assessment activities for children are designed to fit in with club activities so they should not be difficult to implement. Because we are looking for what happens over time, some of these activities will need to be completed at the beginning of your clubs and some at the end. The timing of when to complete instruments is indicated in the table of forms.

Some forms are web-based. They can be completed directly by you and your children on the web. Other forms are just print-based, which means you can print them out from the website, complete them, and then return them via snail mail.
