Explanation
and Consent Forms for Teachers, Students, and Parents
Below
are explanations and consent forms for SAW participants. The HTML
page will open in a new window from which you can select print
from the file menu of your browser. The Adobe Acrobat format opens
in the Acrobat Reader and requires the useful and free Acrobat
Reader plug-in. The
plug-in will open and you can print from the Reader. You can also
save Acrobat files to disk and then open and print.
Please
read and print out all of the forms. Sign the teacher consent
form and make a copy for your records. Then make as many copies
of the Parent/Student Project Explanations and consent forms as
you need for your students. Ask your students and their parents
to read and fill in the forms and return them to you as soon as
possible. Remember to make copies of all forms for your own records.
